Preserve Your Smile After a Failed Root Canal
Endodontic surgery becomes necessary when a tooth that has already undergone root canal therapy continues to show signs of infection, inflammation, or pain. In some cases, microscopic fractures, hidden canals, or persistent bacteria may prevent the tooth from healing. When root canal retreatment isn’t possible or has already failed, endodontic surgery offers a highly effective alternative to extraction.
Also known as apical microsurgery or apicoectomy, this advanced procedure involves making a small incision near the affected tooth to access the root tip, remove any infected tissue, and seal the end of the root to prevent further problems. Using specialized instruments and a surgical microscope, your endodontist seals the root end to prevent reinfection. The goal is to save natural teeth that might otherwise require extraction.
Common Reasons for Endodontic Surgery:
- Persistent infection after root canal treatment
- Hidden or calcified canals that cannot be accessed
- Fractures or cracks near the root tip
- Inflammation or cysts around the root
- Root damage from previous dental procedures
- Blocked canals preventing root canal retreatment
- Tooth anatomy that complicates non-surgical approaches
- Need for a biopsy of the surrounding tissue
The Consequence of Avoiding Endodontic Surgery
If you delay or avoid endodontic surgery when it’s recommended, the infection in your tooth can continue to spread, leading to bone loss, increased pain, and eventual tooth loss. What could have been a minor procedure may turn into a more complex problem. At ReGen Endodontics, our goal is to help you preserve your natural tooth and avoid the need for extraction. Early intervention gives you the best chance to preserve your long-term oral health.
Your Endodontic Surgery Experience
Consultation
Your experience begins with a comprehensive evaluation by our endodontists. Using advanced imaging, typically a 3D CBCT scan, we assess the affected tooth, surrounding bone, and sinus or nerve proximity. This helps us pinpoint the source of infection and determine whether microsurgery is the most effective solution. You’ll receive a detailed explanation of the procedure, risks, benefits, and what to expect, with plenty of time to ask questions.
Treatment
On the day of your procedure, we ensure your comfort with local anesthesia and optional sedation if needed. Using a Zeiss operating microscope and microsurgical instruments, a small incision is made in your gum near the affected tooth. We gently retract the tissue to expose the root tip, remove infected tissue, and carefully excise 2-3 mm of the root end. The root canal is then sealed with a biocompatible material to prevent future infection. The area is then cleaned, sutured, and checked thoroughly before you leave.
Recovery
Most patients experience mild soreness and swelling for a few days, which can be managed with over-the-counter pain medication. You’ll be given detailed post-op instructions, including dietary guidelines (soft, non-irritating foods) and activity restrictions. The sutures are typically removed one week after the procedure, and we schedule follow-ups to monitor the healing process. Full bone regeneration can take several months, but soft tissue heals quickly.

Endodontic Surgery FAQs
Is the Procedure Painful?
No. We use effective local anesthesia to numb the area, and we also offer sedation options if you feel anxious. Most patients report that the experience is easier than they expected and experience only mild soreness afterward, similar to a routine dental procedure.
How Long Does the Surgery Take?
An endodontic microsurgery procedure typically takes 60 to 90 minutes, depending on the tooth’s location and complexity. Molars and teeth with limited access may take a bit longer, but we’ll give you a time estimate during your consultation.
How Successful is Endodontic Surgery?
Endodontic microsurgery has a high success rate, often exceeding 90%, particularly when performed using advanced technology and techniques. Long-term follow-up is important, and we’ll monitor your healing to ensure the best possible outcome.
